The focus is set on forms which are filled interactively on screen and printed. You need a \LaTeX{} distribution, pdflatex, hyperref and insdljs must be installed. The insdljs package is a part of the AcroTeX Education Bundle, see \url{http://www.acrotex.net}. You should have the following additional documentation available: \begin{itemize} \item the hyperref manual: \cite{hyperref:manual}, \item guides, specifications and references for Acrobat JavaScript: \cite{pdf:javascript}, \cite{pdf:scripting:guide} and \cite{pdf:scripting:reference} and \item the \textsc{pdf} file format reference: \cite{pdf:reference}.
